Building Confidence in Stroke Care at Home

0000-00-00

Caring for someone after a stroke can be overwhelming, particularly when families are learning how to manage changes in movement, medication, nutrition, and daily activities at home.
As part of our home-based care program, Compassionate Care for All (CCFA) is working with a stroke patient and caregiver to build their understanding and confidence in managing recovery at home.
The training begins with helping the caregiver understand what a stroke is, how it can affect the body, and why continued care and monitoring are important after hospital discharge. The caregiver is also guided on medication management, including the importance of following the prescribed treatment plan, maintaining medication routines, and recognizing when concerns should be referred to a healthcare professional.
The session also focuses on rehabilitation. The patient is supported with physiotherapy exercises targeting the affected arm and foot, with the caregiver learning how to safely support the exercises as part of the patient's ongoing care routine.
The caregiver is guided on positioning, movement, safe assistance, and the importance of maintaining appropriate activity while considering the patient's individual abilities and limitations. The aim is to help the family understand that rehabilitation is not limited to a facility or a single therapy session. Appropriate exercises and caregiver support can form part of the patient's continuing recovery journey at home.
The training also provides an opportunity to discuss other important aspects of stroke care, including nutrition, prevention of complications, personal care, mobility, safety at home, and recognizing changes that may require medical attention.
For CCFA, this is what home-based care is about: equipping families with practical knowledge and skills so that they can participate more confidently in the care and rehabilitation of their loved ones.
This training is part of our broader commitment to bridging the gap between hospital discharge and recovery at home through caregiver education, rehabilitation support, nutrition guidance, health monitoring, and continued follow-up.
