Managing both diabetes and hypertension can be overwhelming, especially when the responsibility extends beyond the patient to the family members who provide daily care.
When one of our clients enrolled in the CCFA programme, the challenge was not simply remembering to take medication. Daily decisions around food preparation, monitoring symptoms, clinic appointments, and recognizing warning signs had become a source of anxiety for both the patient and caregiver. Despite their best efforts, they lacked practical knowledge on how to safely manage these conditions at home.
Through home visits and caregiver education, the family received guidance on:
• medication adherence
• blood pressure and blood sugar monitoring
• foot care
• healthy meal planning
• Identifying symptoms that required urgent medical attention.
• Caregiver training also focused on building confidence
• Establishing simple routines that could be sustained within the realities of their household.
Small changes gradually became part of everyday life. Meals became more balanced, medications were taken more consistently, and the caregiver felt better equipped to recognize early signs of complications. What had once felt confusing and intimidating became more manageable through knowledge, support, and ongoing follow-up.
This story reflects the reality faced by many families living with chronic illnesses. Effective care is not only about prescriptions and clinic visits. It also depends on the ability of caregivers to make informed decisions every day.
